How to Refer a Child or Young Person to the Children & Young People’s Therapy Service
Our Children and Young People’s Therapy Service accepts referrals from any professional or parent who has concerns about a child or young person’s communication, mobility, physical development, or daily functional skills. [somersetft.nhs.uk]
How to Make a Referral
You can refer a child in one of two ways:
1. Online Referral
Complete the online self‑referral form. Please ensure you have all required information ready as the form times out after 20 minutes. [somersetft.nhs.uk]
2. Referral Forms (Paper or PDF)
The previous referral form may still be used but must be submitted with all additional required documents:
- Children and Young People’s Therapy Service Referral Form
- Children’s Community Nursing Team Holiday/Short-Term Referral Form (where applicable) [somersetft.nhs.uk]
Additional Information Required
To help therapists assess the referral effectively, please include the following documents where relevant:
Speech & Language Therapy
- Communication Competencies Form
- Speech Sound Screener (for children over age 3 with pronunciation concerns)
- Feeding Referral Additional Information Form (for eating/drinking concerns) [somersetft.nhs.uk]
Occupational Therapy
- OT Additional Information Form (school‑aged children)
- OT Early Years Additional Information Form (pre‑school children) [somersetft.nhs.uk]
Completing the correct form ensures therapists receive valuable information for planning the child’s assessment.

Please note:
- Do not send encrypted emails from personal accounts (they become quarantined).
